Local 901 Meal Reimbursement Form

This form is to ask Local 901 for meal reimbursement in the case of dietary restrictions when attending MAPE events or gatherings where food is provided. 

Based on this form, the MAPE Local 901 Executive Committee will approve or deny this reimbursement request. All reimbursements MUST be pre-approved.
